Guidance and application form to be completed by schools appointing a new teacher in any capacity and authorised by the relevant Employing Authority.

Note that a TR268 (TR268P for principals or vice-principals) form should also be completed.

Any teacher appointed on a pro-rata basis must complete a TR142 form.
