Useful forms for teachers
In this section you will find frequently requested forms in relation to teachers' pay. You may print the form, complete it, include where necessary the appropriate authorisation prior to submitting the form to Teachers' Pay Team.

Teacher absence forms
This form should be used to certify illness for the first seven calendar days.
This form(s) should be completed by ALL teachers absent owing to maternity whether or not they are entitled to payment or are resigning.
Forms to be completed should you want to take time off work for paternity/adoptive reasons.
This form should be completed by ALL teachers wishing to claim up to 10 keeping in touch days during maternity leave to ease return to work.
Teaching appointment forms
This form should be completed by schools appointing a new teacher in any capacity and authorised by the relevant Employing Authority. A TR268 (TR268P for principals) form should also be completed. Any teacher appointed on a pro-rata basis must complete a TR142.
This form should be completed by the employer
This form should be completed by schools appointing a part-time teacher and authorised by the Employing Authority. A TR23 form should also be completed and authorised.
This form should be completed by schools who wish to temporarily vary the contract of a Teacher. e.g., reduction of hours, job share.
Salary forms
This form should be completed when appointing a new teacher. It should also be used when awarding teaching allowance, special needs and/or recruitment/retention allowance points.
This form should be completed when regrading Principal and Vice Principal salary placement.
This form should be completed when authorising a payment other than salary.
This form should be completed when authorising a redundancy request.
This from should be completed when authorising an honoraria payment. Please note that this form should not be submitted to Teachers Pay and Pensions Team until formal DE/DFP approval for the honoraria has been obtained.
This form should be completed by pro rata teachers wishing to claim additional hours to their existing contracted hours.
This form should be completed when applying for a re-organisation allowance.
This form should be completed by all new teachers following completion of their GTCNI registration to provide DE’s Teachers’ Pay and Pensions Team (TPPT) with all of the required personal information from the teacher directly in order for TPPT to set up a shell record on the DE Teachers Pay payroll system pending NISTR bookings. It is the teachers’ responsibility to complete and return the TR25 form to TPPT. If TPPT are not in receipt of the TR25 form, this will delay payment to the teacher even if the school/service approve the booking on NISTR. It will be the teachers’ responsibility to ensure that TPPT are informed of any changes linked to their name, address, bank details etc using the contact details in Section 7. TPPT will use the details provided on the original TR25 form unless advised otherwise by the teacher. A teacher will only be required to complete a TR25 once for the duration of their active GTCNI registration.
